From your first call to follow-up, we manage every step of your care.

What to Expect

    Reception

    Our reception staff is ready to welcome patients in a professional and friendly manner. Upon arriving at the clinic, patients should check in at the front desk.

    Check-In

    Our goal is to make the check-in process smooth and efficient. Patients may be seen without requiring an appointment. Walk-in patients are generally seen in the order they check in, although medical emergencies may take priority at the provider's discretion. New patients will be asked to complete appropriate registration and privacy forms, including applicable Protected Health Information and Notice of Privacy Practices acknowledgement forms. Existing patients who have not visited the clinic recently, or who have changes to their insurance or contact information, may also be asked to update their forms. Where available, patients may complete forms in advance to help reduce check-in time.

    • Verify or copy the patient's photo identification
    • Verify or copy the patient's insurance card
    • Collect applicable co-pays or deposits

    Triage

    Following check-in, a medical assistant will collect important information that provides baseline detail to assist the healthcare provider in evaluating your condition and determining appropriate treatment.

    • Brief medical history
    • Current medications
    • Reason for the visit
    • Vital signs
  4. 4

    Provider Experience

    After triage, the healthcare provider will review the information collected, ask additional questions about your health and reason for the visit, and conduct an appropriate examination. When medically necessary, additional testing may be ordered. The provider will discuss applicable test results, diagnosis and treatment recommendations with you, and patients are encouraged to ask questions about their diagnosis, testing and treatment.

    • Blood tests
    • EKGs
    • Other appropriate diagnostic testing

    Discharge & Follow-Up

    Before leaving, patients will receive appropriate information about their visit. If additional testing or treatment is recommended, our staff will communicate applicable costs according to the practice's current policies.

    • A summary of the visit
    • Treatment recommendations
    • Follow-up instructions
    • Prescribed medications, when applicable
