What to Bring to Your Visit
Bring these items with you so we can care for you quickly.
Photo Identification
Patients should bring a valid form of photo identification when checking in.
- ◆Driver's license
- ◆Passport
- ◆Military ID
- ◆Employment identification
- ◆School identification
Insurance Card
Patients using insurance should bring their current insurance card.
Minor Patients
Patients under age 18 should be accompanied by a parent or legal guardian. If someone other than the parent or legal guardian accompanies the minor, appropriate authorization to consent to treatment may be required according to the practice's current policies.
- ◆The accompanying adult should be prepared to provide appropriate identification and insurance information
Medications
So we have a complete picture of your care, please bring either:
- ◆A current list of all medications, including dosage information when possible
- ◆Or the medications themselves
